The Fallen Altar is currently an endgame item used to unlock Incursions. It's only craftable after obtaining the Gateway Tablet.
Upon placing, add the Gateway Tablet into the Fallen Altar to access the Incursions menu at anytime.
Various perks can be unlocked in the Incursion Perk Tree, using Altar Dust. Perks unlocked in a Fallen Altar do not affect other altars, they are only active on their own altar's Incursion. Destroying a Fallen Altar does not affects its perk tree, meaning it will be the same once the altar is placed again.
